It does not replace emergency care or a physician’s evaluation.
How to request a referral
- Describe the general reason for the request and the patient’s Miami-Dade ZIP code.
- Provide the preferred language, emergency contact, and referring provider or facility.
- Ask what order, records, assessment, authorization, or secure document process is required.
