Physicians, hospitals, discharge planners, and other referral partners play an important role in helping patients transition safely to care at home. Ameri-Care Professional Service, Inc. provides a Miami-Dade referral pathway for administrative coordination and review of requested home health services.

A referral is not an automatic admission, coverage decision, staffing commitment, or guarantee of a start date. Ameri-Care must review the request, applicable orders and documentation, patient location, payer requirements, service needs, and ability to safely accept the referral.

Information to include

• Patient name and reliable contact or authorized representative
• Complete service address, apartment or unit, ZIP code, and access instructions
• Reason for referral and requested discipline
• Ordering or referring professional and facility information
• Discharge date or requested start-of-care context
• Available orders, clinical documentation, therapy recommendations, and payer information
• Preferred language and accessibility or communication needs

Secure referral process

Use the Patient Referral page to begin the administrative request and wait for the approved secure channel before transmitting protected records. Do not place full medical records, diagnoses, photographs, government identification, insurance member numbers, or other sensitive information in a public chat or ordinary email.

Ameri-Care may receive referrals from Miami Lakes, Hialeah, Doral, Miami Springs, Coral Gables, Kendall, Homestead, Miami Beach, and other Miami-Dade communities. Service availability and acceptance must be confirmed for the exact patient address and requested service.
