Ameri-Care Professional Service, Inc. provides a Miami-Dade referral pathway for administrative coordination and review of requested home health services.
A referral is not an automatic admission, coverage decision, staffing commitment, or guarantee of a start date. Ameri-Care must review the request, applicable orders and documentation, patient location, payer requirements, service needs, and ability to safely accept the referral.

- Complete service address, apartment or unit, ZIP code, and access instructions
- Reason for referral and requested discipline
- Ordering or referring professional and facility information
- Discharge date or requested start-of-care context
- Available orders, clinical documentation, therapy recommendations, and payer information
- Preferred language and accessibility or communication needs
Secure referral process

Do not place full medical records, diagnoses, photographs, government identification, insurance member numbers, or other sensitive information in a public chat or ordinary email.
